or bringing the form to the SCRAPS 9-Lives Boutique during store hours

If adopting a cat from Excalipurr Cat Cafe, use this form. Fill in and return to SCRAPS to start the adoption process.
If adopting a cat under 6 months of age, a spay or neuter contract will be required.
